So i had a new patient appointment with Jacob Kozacki today at 8:20 and i was referred to him by his mom Krista who my family has been patients of hers for years. gps had me lost and called and explained the situation and the lady on the phone told me that he has a 10min window when usually everyone gives you a 15 min grace period but she told me to come anyway and when i got there at 832 i was told that i had to reschedule which i already had to wait 3months for and use pto for and next available appointment was feb. I would recommend this doctor to no one. He didn't listen to me. Imy wife was misdiagnosed. He brush it off I had mental issue. Instead my wife had life threatening issues. ISbe found another doctor. Who would listen. The time they found out what wrong with her. . She had several stroke. I wish Dr. Kozacki had listen to her. Some her health issues could been prevent.
My Primary Doctor moved and I needed a new Doctor. Someone suggested Dr Kozacki. And he has been wonderful. Listens to you. Ask questions. I have found my new primary Doctor. Thank you Dr Kozacki for being so nice.

He is currently my doctor I first meet him October 11, 2024 he got to meet my fiancée Tyler and my sweet 3 year old babygirl.
