I reached out a week ago to get records information only to receive a call today from a male asking me to fill out a release of information form. Which is fine. What isn't fine is during the initial call I was told those records would be forwarded in 72 hours. This follow up call occurred today. THE DAY I needed my records by. The individual on the phone asked me who I had spoken with. The woman who I chatted with did not identify herself as front desk or call center (who does even when working front desk???).
