My family is new to GA, my family have been seeing this dental office for the past yr under the same insurance. Finally I got a chance to visit them 1st time, I filled out new patients forms but was called upon saying I need to provide my SS#! They’ve been operating without my husband or my SS# for the past visits, all in a sudden that becomes a problem?
