The accompanying literature implies affiliation with our insurance provider and that the tests are sent in cooperation with our insurance provider A. I mailed my urine test kit per instructions. I have called and left messages for customer service four times with no response. I am unable to obtain my test results. The website requires a password which has no provisions for creating one. The sample code number is also useless.
