Beware! I just started receiving Medicaid and needed a cleaning, so I had to choose a dentist who accepts Medicaid, and Dr. Doan @ Dentist for Life was on the list. Before even being seen by the DDS, I was asked to sign consent for billing for a mouth guard, flouride treatment and numerous x rays. I had not asked for any of that and did not want a flouride treatment or a mouth guard so I questioned why that was listed and why it was included on the bill without me ever being consulted or seen by the DDS first.
