As requested by Hanosh & Hunter, I submitted new patient forms online including PPI and waited for someone to call me back to book an appointment. I waited two weeks before I called back to follow up for an appointment and they told me yes, they remember my info coming through but need to verify insurance info and call me back. I waited over a week again for someone to call me back but they never did so I called again for follow up.
