This office is unprofessional and so is the doctor. He has to be aware of how he handles situations and his staff. A week ago a patient asked to have 2015 form filled out for transportation to be accepted thru the insurance of the patient ! Doctor claimed his office is not doing those forms till January without a explanation of why! Meanwhile this form is filled out in less than 5 mins online.
