I was recently there visiting a loved one. My plan is to report them to the board of health. I have documented evidence of neglect and filth. Shut it down.

Awful place to go to. They push the person out of rehab with out having help at home. Only 1 good nurse there and that was Gina and social workers were awful. Treated the veteran with disrespect. Go some place else. Warning

I am deeply disturbed by the substandard care my loved one received during her stay at this facility. The nursing staff's behavior was unprofessional and inhumane, as evidenced by their derogatory remarks towards my loved one's roommate, who had suffered a stroke. This thoughtless behavior caused the victim undue distress and discomfort. Furthermore, my loved one's call bell was taken away, leaving her without assistance when she required it. The staff at Phoenix Center truly care. I work in the industry at a community near by and interact with this staff on a regular basis. They strive to provide the best for those they serve.

DO NOT SEND YOUR LOVED ONES HERE!!!!!! If i could give 0 stars i 100% would. Only rating it 1 star as that is the lowest i can go unfortunately. Horrible conditions all around. Dirty, mistreated, neglected, outdated…the list goes on.
