I CALLED TO GET A RELEASE FORM FOR MY NEW DOCTOR AND WAS TOLD I HAD TO COME IN THE OFFICE TO SIGN I LIVE 595 MILES AWAY. THE NEW DOCTOR HAS TO HAVE THE RECORDS BEFORE U MAKE THE APPOINTMENT AND MY OTHER 3 DOCTORS THERE I HAD NO PROBLEM WITH THEM SENDING ME THE RELEASE FORMS. I TOLD HER I WOULD GET IT NOTARIES AND SEND IT BACK TO HER SHE SAID CAN'T DO THAT I HAVE TO SIGN THEM IN FRONT OF HER.THAT IS NOT RIGHT BUT I WILLSEND THEM THE INFORMATION AND CAll A LAWYER IF THEY DONT SEND MY INFORMATION.
