Dr. Padberg was my dentist for 30 years. I am really shocked and saddened to make this report after all this time. I walked in for my appointment, and I was told I needed to fill out some paperwork. I asked the receptionist what it was for, and she said it was a medical history. I asked them why after 30 years, they now need this paperwork. I was told that they were a medical office and they needed the paperwork. I told them that they were not a medical office, but as a dentist, they are a health care provider.
