The staff was amazing. Came for rehab but ended up having to have hospice come in and graduate my family member on to heaven. Team worked great together and we had the best experience ever here!! High recommend

The level of quality / dignity service has declined drastically in this location. We had My 93 year old mother stay here twice. Once for rehab and another for respite care for 7 days. I could not believe that the level of service had changed so drastically. One could say they were either short staffed or just stopped providing the same quality they use to. My experience with Forum Parkway Health & Rehabilitation has been extremely disappointing and deeply concerning. This facility is supposed to provide care, comfort, and support for residents, but what I experienced was the exact opposite. The administrator, Dylan Gadberry, is very dismissive. It is clear that he does not truly care about the concerns of families or the well-being of the residents. The facility is nice and well kept. My 82 year old dad was at this facility approximately 3 months. My family didn't have many facilities to choose from since my dad was a trach patient. We chose this facility due to location, and it was well kept. The staff did an okay job. Some that stood out and went above and beyond for my dad were: CNAs (Torina and Pride). They do awesome work! This facility is harassing me because my daughter put my name down as her emergency Contact. Now they are sending me a bill and threatening to send it to collections. I called them to explain that I have never used their facility and they rudely hung up on me. So be careful when you visit a friend, they will send you the bill.
