I booked an appointment with this provider, who IS within my preferred MODA Health network. They had me fill out four pages of paperwork before my first appointment that took OVER an hour. The day of the appointment, Tom the receptionist called to let me know I had a deductible I had not met. I said I was aware of it, and thanked him for explaining details of the health plan I signed up for (and was fully aware of).
