I do not recommend this office. I have multiple physical skeletal deformities, that are on file, X-rays and other documentation. These issues impede my ability for certain activities such as walking excessively. I needed an ADA form filled out from Dr. Suzanne Putnam. The form was intended to provide documentation that I would need to use a fork lift in performing my job to lessen walking. Dr. Putnam faxed the form 3 separate times incomplete each time and with inadequate answers to the questions she did fill out. 3 months o... read more

clean and well presented from the exterior
I've been Dr. Putnam's patient for quite a while. She is easy to talk to and careful with her patients. I feel very blessed to have her...very! Dr. Putnam is a great doctor and has a great team.
Dr Putnam has been my doctor for at 15 years she is the doctor i have ever had. i give her a 5 star rating. thank you Dr Putnam for the great work you have done for me at saltsburg family practice.
