The most caring providers! You’re in great hands
this practice and danielle jasinski specifically are affecting my friends employment because they say they send forms in a timely manner and they lie. When calling for confirmation the form was never sent out, in fact it’s siting on her desk and she’s out of office. This is ridiculous, they do not care about patient health.
The constant turnover of doctors and providers at Evergreen is a major red flag. I wish I had read the reviews sooner. If you’re considering visiting this office—DON’T. They do not care about their patients; their only priority is making money. I am completely disappointed and dissatisfied with their services. During my annual physical last August, which I scheduled knowing my insurance would cover it, my provider asked a general question about how I was feeling. Being younger and in generally good health, I don't have terribly high expectations of my primary care physician. This makes it all the more upsetting that the one time I needed them for a particularly nasty sinus infection they couldn't be bothered to deliver even a mediocre standard of care. Perhaps the turnover of Doctors/Providers at Evergreen is enough of a negative review in itself. I've been a patient at Evergreen for around 6 years. During this time, my PCP left the practice. I have explained many times I would prefer (and will only) see a female provider. My original PCP was replaced by a female provider. I saw her once and she left the practice.
