The surgery went great, the doctor was awesome. BUT… i paid out of pocket and they are refusing to send my Ins Co a claim form filled out so i can be reimbursed. You see, they told me if i wanted to have the surgery at the time i made appointment it would have to be out of pocket or if i wanted to go through my Ins. Co. they would schedule another appointment at a later date and this appointment would be a consultation.
