I called the office on May 19th to get a living will done. Secretary advised she would email me some forms to fill out and someone would contact me soon. Forms were emailed to me and completed on 05/20/2021. No one called or emailed me for several weeks so I called the office on 06/11/2021. I asked about the service I requested and she did not know what I was talking about. I advised her forms were sent to me on 05/20 and were emailed back that same day.
