In perusing the website, I liked the convenient online appointment request form and filled it out. When I received a follow-up email, Kari requested duplicate information (DOB, phone, diagnosis) as well as for a diagnosis. Her reply even contained a copy of my online submission, so the information was in the very email requesting it.
