I have been at patient of Clarkson for 4+ years. I received an email stating I needed to make my annual appointment. I made my appointment when I was at my appointment last year. I called to confirm I had an annual appointment and was told it had been canceled. I did not cancel the appointment. So I asked to have my appointment scheduled. I was asked to provide my insurance companies, numbers, etc. I said you have my records I can verify them. I was told I had to restate all of my information.
