Horrific practice! The USCIS sent a letter requesting additional evidence due to an expired I-693 form on my I-458 application. The front desk agents dared to slander that it was my lawyer who sent the letter late ( with dates of last year) or the USCIS immigrant processed it late ( again last year). The letter clearly stated the form received was outdated (2 years + expired) form. They denied the mishap and insisted that it was not their fault.
