My mother has been a long-time patient at this office. The service here used to be outstanding, but her recent experiences have changed my perception. She submitted a form to be filled out and waited five weeks for it to be completed, only to find out that it had been lost. She then provided a second copy for completion. It has now been six weeks since she dropped off the second form, and despite making three phone calls to check on its status, no one has returned her calls.
