If I could leave negative stars I would. We went to this doctor for the required exams for immigration paperwork - she's listed under USCIS's approved civil surgeons. They give you a sealed envelope that you have to deliver to USCIS. Well. When we went in for our interview, it turns out that the doctor failed to put the date on the form, next to her signature.
