I was recommended to Dr. Kennedy by a friend. I called the office to see what all it takes to get an appointment as a new patient. I was told to have my records faxed from my primary doctor's office to his by the lady on the phone. My primary's doctor office requested I reach out to his office to get a records request form. I did call his office back for this form and was told I'd have to pick it up from his office. The form wasn't available online and no offer to fax the form.
