Asked her if she took my insurance. She did not answer directly, but quoted OON rates. So, I guess she doesn't take my insurance. But then I check my provider's website. It says she's in-network, but the rates are different for a home/office location vs. facility/hospital. So I asked her, how are you coding your treatment location?
