Walked into this dingy dirty office, with lawn chairs in their waiting room! This place has at least a 30 page questionnaire. It asks at least 10 times your alcohol and drug use. It never really focused on your medical history just your abuse history! Unless you are an addict of some sort....DO NOT GO TO THIS FACILITY!!!!
