Did this provider ever take personal phone calls while in the appointment room with you? No, they've never taken personal calls with me Does this provider always put the customer first? Absolutely! Yes, it is very conveniently located Was it easy to find parking at this provider's office? Yes, it was convenient Was this provider friendly? Yes Did this provider wear an obnoxious cologne? No, they didn't wear any cologne

Does this provider ever overbook appointments here? All the time! the waiting rooms are always packed and It seems as if they are more concerned about quantity over quality Does this provider leave you on hold for a long time when you call? Yes, it never fails. I'm put on hold forever when I call Did you leave the office feeling satisfied with your visit? No, I felt confused and uneasy when I left

Does this provider use current and safe equipment? Yes; the equipment is very advanced! Did a staff member acknowledge or greet you when you entered? Briefly, but they didn't make an effort to help me Does this provider welcome questions? Yes, they don't mind answering my questions
