This is part of their initial paperwork packet and listed as an "opt in" portion of their care. When I went to my appointment, I filled out all the paperwork except for this (as it was an OPT IN document) and I chose not to "opt in". I am a new patient of Dr. Sajjad she is an amazing doctor. She spent the time to go over my history and helped get me all my issues addressed. Her office staff was also very kind from beginning to end. Would highly recommend this office and both Doctors here!!!!
1st Visit intake was awesome: front end staff, her MA, and Madiha Sajjad, MD., were extra precise and helpful. Glad to have found a competent and PCP Transferring care was easy.
