Unprofessional. Had an appointment scheduled for over 6 months and he left the practice forcing me to schedule another appointment at a different practice over 6 months out. Will be avoiding any practice that he is employed at.
When I arrived at what I was told was the location of the appointment, no one was there. I then found that I had received an email the day before, telling me that the location had been changed to one 20 minutes away. I was also told that I had failed to fill out all of the prerequisite forms, despite having contacted the doctor days beforehand specifically to ensure that I had all of the forms in order, and being told in no uncertain terms that I had done everything that was required of me.
