This location lost my blood work but still charged me, had to stick me with a needle multiple times (even switching arms) because the nurse kept missing the vein, failed to follow up with me about whatever findings they could gather from the half labs they performed, and will not answer the phone. This place is an absolute joke of a doctor's office and I PROMISE you will be better off going anywhere else.

This office is a fraud, as others have stated. DO NOT COME HERE. I made an appointment through ZocDoc with Dr. Charochak, but was told upon checking in that I'd be seeing his PA instead. This was upsetting, since I was looking to establish care with a GP. There were two medical assistants who took me back to the room, with one clearly being in training. Absolutely a fraud. This office doesn't even have a doctor on site. This is a physical therapy place passing off as a doctors office. There is no communication with the office. Don't believe me? Try calling the listing now to set up an appointment. The only way they schedule appointments is through online schedulers like ZocDoc or their website. Their reviews are all hand picked to inflate their status. DO NOT COME HERE FOR AN APPOINTMENT!
Very dissatisfied with my experience here! Went to see Dr. Charochak at this office, but found out that he doesn't actually see patients. He is a figurehead of the office only. The "physician" I saw didn't have an M.D. and didn't help me with my problem. It was very clear that they are only trying to sell added services (physical therapy next door), not trying to help determine the root cause of spinal issues.
