I was sent to this Dr. from Social Security for a neuro evaluation. After reaching my case worker, she told me the Dr. never sent in most of his paperwork evaluating me. I lost my home, my new car, my job, after my son passed away. I had to borrow money to drive 20 miles for this visit and it put me behind another two months. I've waited a year.
