Went to do blood work. Facility did not have authorization. Called office and gave fax#. Went the next day for blood work. Orders that were faxed did not have codes. Again no blood work done. Totally unacceptable. Furthermore we have not actually seen Dr. Azem in a year just his medical assistant. For the cost of healthcare we should see the doctor at least once a year. We may be looking for another doctor.
