Stay away from Dr. Hafez Azadeh and his shady billing practices. I had surgery and a hospital stay in 2006 which was mostly covered by insurance, with all outstanding bills being paid shortly thereafter. Out of the blue in 2008, two years after my surgery and without ever hearing anything about a Dr. Azadeh in the past, this doctor sent me a bill for services he says were rendered in 2006.
