The address is wrong. Call and check the actual one. Waited more than an hour to find that receptionist can’t verify my insurance. Called insurance to hang on the line forever to explain they need to fax my benefits. And they said they will fax same thing written on my card. Lazy yawning receptionist was whispering that it’s office manager request and she can’t do anything. Office manager never came to talk to me. Spent about two hours for nothing. I’ll just go find some other doctor who can read what insurance card says .
