My I-693 immigration vaccination paperwork was DENIED citing "cannot confirm the validity of the Dr's signature". I paid for my vaccine and the completion of this document for my immigration application March 2023. I am now aware that Dr Eloi has passed away but I am still now left with no valid vaccination record and have to pay another civil surgeon to complete this and possibly have to take the vaccines AGAIN as they did not provide me with a copy.
This is my first time with this Doctor what a disgusting experience! I couldn't find the location I was in the proper plaza but did not get a suite number so I was calling them it was 12:40PM. My appointment was 12:45 I kept calling to find fet the suite number but no one answers the phone! Sitting outside calling over & over no answer. la secretaria es muy mala persona, no me trató bien en mi primera visita al médico, estaba estresada todo el tiempo que mi hijo de dos años estaba revisando los papeles que había organizado y yo iba a Organizar las tarjetas de presentación tan pronto como terminé de llenar los papeles.
