Office staff on rt 1 wanted me to sign an incomplete form and was annoyed that I even asked to read it first. She stated the reason she hadnt filled out the form prior to signing was that it was easier for her and that if I didn't sign it they wouldn't treat me. I replied that my attorney would be upset if I signed an incomplete form( workers comp case). Very disappointed on care I received had my cortisone shot at salem hospital. While I was laying on table all i heard was him arguing about staff hospital and how he would never work there again Today i had phone appointment asking for a second shot because first one didn't work.
