Worst doctor ever. Was hospitalized just needed a basic form with a work ETA for an office job for my disability insurance. Invited him to write in his own date. I was released from his charge with the stipulation that I not operate vehicles. I love in New York so this was not a problem. He told me — using a ridiculous defensive and argumentative tone of voice —he wouldn’t sign the form because “I wasn’t coming back to Ohio to see him.” Huh? I’ll get any other doctor to sign off.
