This place is a joke! I made an appointment back in March via phone. Their earliest availability was June 11th. I took it and accepted that I would need to be patient. The office contacted me last week to fill out the new patient forms. I spent 15 minutes answering questions on survey after survey, and uploaded all necessary documents to prepare for the appointment. They texted me asking me to confirm the appointment.
