Will not provide simple information about my CURT enrollment information. All you get is a screenshot with very little info on it. If CURT enrollment is part of your medical records, then wouldn't it be illegal to withhold information regarding that enrollment.
All I can say is do not hire these people..........also own Meadows by the lake AFC ..do reshearch on state complaints.

I registered for an appointment, submitted my online forms, and joined my virtual session. However, no provider joined my session. I called the phone number but no one answered, so I left a voicemail. It's frustrating because I took time off work to make the appointment. [Update: The provider texted me 25 minutes after my scheduled appointment start time to apologize and explain that they were running behind. Updating my 1-star review to 3 stars.]
