Do not I repeat do not go see this “doctor” or lack there of… Normal doctors care about their patients….. so I asked Dr Smith My prescribing doctor to fill out a basic 391.41 CMV drivers medication form… i to operate a small dump truck DOT requires the prescribing doctor to fill out every 2 years. . The form Is literally four questions.. All basic questions that the prescribing doctor, should know the answer to... Even the DOT doctor that gave me the physical said why is your dr prescribing a medication that he can’t answer ... read more
