The doctor didn't give me the paper documents with her signature. She told me that everything with her signature in my chart. There was no such document in Mychart.
Dr. Butromowicz is great. She's professional and thorough at what she does as a doctor. She also is funny and engaging about the world outside the office despite her packed schedule. I had sinus surgery at UCSF in SF by one of her colleagues and having her to do follow ups close to me in Berkeley is very convenient. Highly recommended.

Dr. Butrymowicz aka Dr. B is caring, professional, informative, and goes the extra mile for her patients. Her thoroughness is bringing to me the medical care I need to cope with the debilitating symptoms from a traumatic brain injury. My life has changed like I never imagined in the past year.
