I went for a well women’s exam at Dr. Anisha Farishta’s office. Dr. Farishta told me since I had a close female relative who had breast cancer at an early age, I needed to do this survey that would determine if I had an increased risk of cancer. I go to the front desk after my exam, scan the QR code for the survey, take it, then show my results to the lady at the front desk.
