This doctor, after approximately 15 years a my primary care provider, terminated our professional relationship in writing by certified mail instead of authorizing a lab test to check for Covid antibodies in my blood, which I would have paid for out of pocket to try to keep my career of nearly 10 years (October 18th, 2021).
