I am chemical sensitive with environmental issues and this doctor had me to sign a form inside his office. I don't know what the form is about. I plan to call them and ask for a copy. This building is very old and if you have been overexposed to mold. This place makes your body jerk uncontrollably and you can not control your reflexes and responses.
