They said on the phone at the time of appointment to fill out a form online the link to which they will send via email. The form was thorough and extensive. It took 20-30 minutes to complete. On arrival though, they asked me to fill the same information again on paper. I refused and the PA did not seem to have any information. They took the copay, but the PA did not examine me at all but gave some general information. So, do not fill the forms online.
