Twelve months and eight requests to receive medical records, informed after procedure insurance was not accepted. At our initial visit I provided my insurance information, filled out health history paperwork, paid and discussed the procedure. Only until after the procedure was completed, was I informed that they do not accept my insurance (MetLife).
