CPT code 99406- is used to bill for intermediate smoking and tobacco-use cessation counseling visits, lasting greater than 3 minutes up to 10 minutes. He inadvertently asked me if smoked, I Mentioned to him that I used smokeless tobacco, He explained I shouldn't, because it raises blood pressure. conversation was less than a minute. Then I got billed an extra $50 for that 1 minute conversation. Didn't meet the requirement to be billed as that code. Perfect example of upcodeing/phantom billing.
