Lisa A. Chartier is the sole reason for this complete crap of a review.. I had an appointment for 11 am. I got a message on MyDH at 8 am requesting I fill out some questionnaires before seeing her(?) that afternoon. Got to it and completed. I got to my appointment early, as requested, for being a ‘new’ patient. Had to fill out the same questionnaires again.
