Dr. Havriliak saw my son in the ER four years ago, when my son needed stitches. His bedside manner was poor, but it was an emergency so we didn't have a choice. Even though Dr. Havriliak was providing service in an ER at an in-network hospital, he submitted an out-of-network claim to the insurance company for over $6,400. Horrible. Do not use this Dr. if you can avoid it, unfortunately this was during an ER visit in which I had no choice. Billed an exorbitant amount for a couple stitches that was not covered by my insurance and then the office refused to work with me regarding the bill. It was difficult to contact anyone in the office to make a follow-up appointments and to resolve an sort of billing issue.

Provided stitches in my daughter's forehead. No bedside manner. My 4 year old was hysterical and had to be wrapped in a swaddle. He made no attempt to calm her or me. Proceeded to bill me beyond what was covered with I believe is against NJ law following an ER visit.

My son was treated by Dr. Havriliak for a few stitches in his head in the emergency room. He was out of network but the only plastic surgeon on call. He treated my son in 10 minutes. We later received a bill for over $8,000. Dr. Havriliak should have let us know how much he was going to submit to our insurance prior to the service if he was going to charge such an astronomical amount of money. We were blind sided. They did not work with us on payment of the balance either.

Terrible. He charged over $8,000 for 3 stitches to the head (in the emergency room) and then refused to work with us to pay the balance that was not covered by our insurance. We were blown away when we received the bill and even more shocked that we were sent a bill for the balance after our insurance company paid $6,000 for the service.

We were so fortunate that Dr. Havriliak was on duty at Hackensack University Hospital when my 21 month old needed stiches above her lip. We just went to his office in suffern to have the stitches removed. He had great bedside manner at both the hospital and the follow up visit. His office staff was friendly and helpful during intake. We are very happy with our decision on having a plastic surgeon perform the stitches on our daughter. I highly recommend Dr. Havriliak.

Dr. Havriliak treated my son @ nyack hospital and is not within our insurance network. Dr. h accepted payment from our insurance in the amount of $1665.00 and billed us, without any forwarding address or phone number for which to pay the bill for a remaining $4995.00. Within a month of the bill, sent a lawyers letter threatening to sue us for a $3320.00 balance. Secretaries at his office are very rude & after my first contact with them to attempt to pay the bill, are refusing any contact with me
