I have been attempting to see a care provider since the beginning of this year . All appointments have been cancelled due to their doctors and NPs not showing up the day I have an appointment. Every time I call to reschedule the earliest appointment is a month and a half out. I have rescheduled about 3 times now. I call and they literally don’t care and sound so annoyed. I am submitting a formal complaint regarding my recent care at Cucamonga Valley Medical Group. I experienced delays in care due to a referral not being properly sent for imaging, which was confirmed by both my insurance provider and RadNet. I was also provided with an incorrect doctor’s note by Office Manager Faye Pacheco that impacted my employment. The girl at the front desk was very rude and very unprofessional & disrespectful, I called in to let them know I would b a little late due to an accident on the freeway and I ended up taking the streets she said to come in, When I got there this girl at the front desk said she would have to reschedule because I didn’t call in so I told her I did call and let the lady who answered the phone no what was going on and she said to come in.
