Coronado Healthcare Center is the worse skilled facility in Phoenix  Az . If you love your loved one DO NOT place them here. This facilty is not attentive to those that are placed in their care. Mom had only been with them about six days. I should have listened to the reviews of patients and not the five star rating from the CMS. They have orders that mom is a fall risk and a floor mat is needed. There is a sign advising that she is a fall risk of five. They did not place one in the floor last night and mom tried to get out... read more
