I was shocked when I read “possible drug user” on my chart in plain site. As an RN, I know that you Never document subjective information. As I questioned Dr. Parris’s nurse about this note she pointed out that she noticed “multiple” prescriptions for narcotics on record which included two pain medications..one for a gallbladder removal surgery and one for a tonsillectomy which were prescribed to me over a period of 3 Years!
